Below you will find all information and forms related to participation in athletic activities in the Collingswood School System.
Information is divided into Board Policy and Regulation and Pre-Participation forms. Parents are encouraged to read through all posted information prior to signing any forms.
Pre-Participation forms must be completed in their entirety before a student is permitted to begin practicing for any athletic activity. All files are in PDF format.
Online Sports Registration and Medical Information
There are no longer any paper forms to register for high school or middle school sports except for the yearly physician’s physical and health history update form. The nurse must receive the completed NJ Department of Education Physical Examination Form, the Clearance Form, and the Health History Form prior to the first tryout or the first practice session. These forms may be printed out from the links below OR through the Family ID sports registration website.
Each athlete must register through Genesis Parent Portal prior to each sport season. After registration is complete, please bring the physical, history form, and/or history update to the nurse.

A physical examination is valid for 365 days from the date of the exam. There are no extensions or exceptions.

Physical Examination Options for Student-Athletes
Option 1: If the athlete has a current physical on file (from a prior sports season) in the nurse’s office, the Health History Update (HHU) form is still needed for each sport season. The “update” must be completed within 90 days of the first tryout or practice session. Please click on the link for the Health History Update Form.
Option 2: A student must have a pre-participation physical examination (PPE) performed by his/her physician. The PPE may ONLY be completed by a licensed physician, advanced practice nurse (APN), or physician assistant (PA) who has completed the Student-Athlete Cardiac Assessment professional development module. It is recommended that you verify that your medical provider has completed this module before scheduling an appointment for a PPE. Please click on the link for the Physical Examination Forms.
Option 3: If a student does not have a health care provider, they may schedule an appointment through the nurse’s office with the school physician (856-962-5742). In order to make an appointment with the school physician, the Health History Section of the Physical Examination Forms must be completed in advance and given to the nurses.
