END OF YEAR REMINDER : Please make arrangements to pick up your child’s medication by 12:00pm on 6/20/2023, as no medication will be stored over the summer and will be picked up by Medical Waste. If you have any questions or concerns, please call the nurse’s office.

Medication must be dropped off and picked up by the parent/guardian. New medication permission forms for each medication are required each school year.

Health Office Update: Please complete the Health History Update form in Genesis for each student upon first login for the year.

Physical Examinations & Immunizations Requirements:  

Yearly Physical Exam: performed within 365 days of the first day of school:MS~HS ATHLETIC PE  

Immunization Requirements: K-Gr.12 Immunization Requirements

*All updated forms due the day after your child's appointment in order to meet compliance*

Medication/Medical Procedure:

All medication must have a healthcare provider’s written order and written parent permission annually.

All medication must be brought to school in original packaging by a parent or guardian.

***All paperwork must be completed prior to any administration of medication/medical procedures.***
